It is an English proverb which basically means that eating nutritious food makes you healthier. As a result, there is no need to visit the doctor very often. I've also heard about another alternative: "Eat an apple on going to bed, And you'll keep the doctor from earning his bread". We even have our Russian equivalent: "кто яблоко в день съедает, у того врач не бывает"
